To be entered in the drawing you must come into the clinic update your and/or your family’s registration with the following information:

* Current Registration
* Updated Picture ID
* Tribal Card or CIB if Alaskan Native / American Indian
* Proof of Income, if eligible need to fill out sliding fee application (last two pay stubs or last years taxes)
* Copy of insurance, Medicaid or Medicare Card

If you are current on this information, you would need to verify with the front desk to be added to the drawing.
